Before Your First Visit

If you make an appointment to visit us for the first time at Hazel Dell Dental Care, we first want to say “Thank you!” We are looking forward to meeting with you and establishing a plan to meet all of your oral health needs. Next, we would like to ask that you fill out the following documents at home and then give them to our staff when you arrive for your initial visit. This will expedite the new patient process.

What To Expect During Your Initial Consultation

During your first visit, we want to get to know you. We’ll review your medical and dental histories and ask if you have any particular problems or concerns. We will take digital x-rays of your teeth and then discuss your options on how to treat any issues. We also encourage you to ask any questions you may have so that you can feel comfortable and informed before choosing a particular procedure or treatment plan.
